How to use the emergency contact sheet printable
- Optionally type a title, a household name and an address to print at the top.
- Choose how many blank contact rows to print, from 0 to 12. Type names, relationships and phone numbers in the contacts list if you want them printed for you.
- Tick or untick the doctors and services table and the medical notes box to fit the page you want.
- Print the sheet or save it as a PDF, then write in the details and any medical notes yourself. Keep it where everyone in the home can find it.
The default sheet
With 8 blank contact rows, the doctors and services table and the medical notes box all switched on, the sheet prints on one US Letter page. The first table has 8 rows for family, friends and neighbors, the second has 6 rows labeled family doctor, dentist, pharmacy, school or work, insurance and landlord or utility, and the notes box is about 2.5 inches tall.
If you type three contacts and ask for 5 blank rows, the table has 8 rows, with the first 3 filled in. At 12 rows the sheet still fits with a shorter notes box, and the tool reports an error instead of shrinking anything if you ask for more.
What goes on the sheet
The sheet is a plain list of who to call and what to know. Most households fill in family members, a neighbor who can reach you quickly, a doctor, a dentist, a pharmacy and the contact for school or work. The services table has labeled rows so nothing is forgotten, and you write the names and numbers in by hand or type them in the tool first.
The medical notes box is blank on purpose. Write allergies, medicines or anything a helper should know in your own words. The tool never suggests medical details, and it does not store what you write on paper.
Keeping it useful
A contact sheet is only as good as its last update, so write the date in the Updated field and review it when numbers change. Put one copy on the fridge or inside a cabinet door, and take a photo or keep a second copy in your wallet or car. Phone numbers you type are kept only in your browser if you choose to remember them, and nothing is uploaded.
For other household lists, the checklist maker and the home maintenance checklist generator make tick lists, the moving checklist generator covers a move, and the pet care schedule keeps a pet's routine in one place.
Assumptions and limits
- The page is US Letter, portrait, and always one page.
- Contact rows run from 0 to 12 in total, including the rows you type.
- The medical notes box is blank and meant to be written by hand.
- Text the PDF fonts cannot draw is flagged in the notes under the preview.
Frequently asked questions
What should an emergency contact sheet include?
Most include family members, a nearby neighbor, doctors, a pharmacy, school or work, and a place for allergies and medicines. Add what matters to your household.
Can I type the contacts instead of writing them?
Yes. Add rows in the contacts list and they print on the sheet. Leave the list empty to print blank rows.
Does the tool store phone numbers?
Only in your own browser, and only if you tick Remember my entries. Nothing is sent to a server.
How many rows fit on the page?
Up to 12 contact rows fit with the doctors and services table and the medical notes box. A message appears if the options do not fit.
