How to use the medication reminder chart
- Give the chart a title, an optional name and an optional first day, which puts a date over each day of the week.
- Choose the times of day that need a box: morning, midday, evening and bedtime.
- Optionally type the medicines, one per line, with a short note after a bar ("Name | with food"). Add blank rows to write in by hand.
- Check the preview, then Download PDF or Print. Tick Remember my entries only if you want what you typed kept in this browser.
What the chart is
The chart is a weekly grid in landscape layout. Each row is a medicine and each day of the week has one small box for every time of day you chose, labeled M for morning, N for midday, E for evening and B for bedtime. You tick a box when you take or give a dose. A key at the bottom explains the letters, and with a first day each weekday also shows its date.
It is a memory aid and a paper record. It does not know what any medicine is, and it does not check doses, timing or interactions.
Use it with your care team's instructions
Fill in the medicines and times exactly as a prescriber or pharmacist told you. This chart does not replace medical advice, and it cannot tell you when to take something, how much, or what to do if a dose is missed. Ask a pharmacist or your care team for those answers. The text under the title reminds anyone who reads the printed sheet.
Nothing on the page is interpreted. Names and notes you type are printed as written. If you do not want to type any health information, leave the list empty and write by hand on blank rows.
Layout and related sheets
About 15 rows fit on a landscape page, and up to 24 rows are allowed across pages. Long names are shortened with three dots and the page tells you, so use the note space for details. For other care records, see the emergency contact sheet, the pet care schedule, the baby feeding and diaper log and the habit tracker.
The PDF is made in your browser and nothing is uploaded. Saved entries are kept in this browser only if you tick the box.
Assumptions and limits
- The chart has 1 to 24 rows and 1 to 4 times of day, seven days across, on US Letter in landscape.
- Medicine names and notes are printed exactly as typed and are never checked or interpreted.
- If you enter a first day, the seven columns count forward from it, and it does not have to be a Monday.
- This is a memory aid and not medical advice. Nothing is uploaded.
Frequently asked questions
Does this chart tell me when to take my medicine?
No. It is a blank grid for you to fill in with the instructions from your prescriber or pharmacist. It does not recommend, check or interpret anything.
Do I have to type my medicines?
No. Leave the list empty and add blank rows to write the names in by hand. Typing is optional and nothing is stored unless you tick the remember box.
Can I use it for a pet or a family member?
Yes. Add the name at the top, and follow the vet's or clinician's instructions for what goes in the rows. The chart treats every row the same way.
How many medicines fit?
About 15 rows fit on a page, and the chart continues on more pages up to 24 rows in all. Each row has up to four boxes a day.
