How to use the permission slip generator
- Enter the school or group and the event or trip, and a short description for families.
- Fill in the date and time, where, cost, transportation, what to bring and who to ask, and a return-by date.
- Choose one or two slips per page, how many copies you need, and whether to add lines for health notes and an emergency contact.
- Check the preview, then download the PDF or print and cut the pages if you chose two per page.
What a slip includes
A slip has three parts. The top explains the event: its name, a short description and the details families need. The middle states the return date. The bottom is the form for the family: the student's name and grade, the parent or guardian's name and phone, two check boxes for yes and no, optional lines for allergies or health notes and an emergency contact, and a signature and date.
With two slips per page, a dashed line runs across the middle for cutting. Long text is shortened with an ellipsis rather than overflowing, and the page tells you when that happens, so you can shorten the text or choose one slip per page for more room.
Check it against your own rules
Rules about what a permission form must contain differ by school, district and state, and some activities need additional waivers or medical forms. This tool is a template. It does not give legal advice, so have the slip reviewed by your school or group before you send it home. Your entries are used only to draw the page in your browser.
For related paperwork, the sign-up sheet generator collects volunteers or chaperones, the letterhead and memo generator writes the cover note, and the travel itinerary generator lays out the schedule for a longer trip.
Printing tips
Print at 100 percent on US Letter paper. A slip on one page is easier to sign and to find in a backpack, and two to a page saves paper for a large class. Ask families to return the slip by a clear date, and keep a copy of your own.
Assumptions and limits
- Slips print on US Letter paper, one or two per page, up to 100 copies per file.
- The slip is a template and carries no legal wording beyond what you enter.
- Long text is shortened to fit and the page says when that happens.
- Health and emergency lines are blank spaces for families to fill in.
Frequently asked questions
What should a permission slip include?
The event name, date, place, cost, how students will travel, a return date, and a place for the parent's name, contact details, choice and signature. This tool prints all of those.
Can I print two slips on one page?
Yes. Choose two per page and a dashed line separates them for cutting.
Is the slip legally valid?
That depends on your school and local rules. The tool prints a template and does not give legal advice, so have it checked before use.
Can families write down allergies on the slip?
Yes. Turn on the allergies or health notes line and the slip prints a space for it.
