How to use the customer feedback form printable
- Type your business name, a form title and a short note for the top of the form.
- Write your questions, one per line, up to eight. Pick 3, 5 or 10 boxes per question and describe the lowest and highest box, for example Poor and Excellent.
- Choose the number of comment lines, whether to add name, contact and date lines, and whether to print one or two forms per page.
- Check the preview, then use Download PDF or Print. Set the number of pages for how many sheets you want.
What the form looks like
Each question is numbered and has a row of numbered boxes beside it, so a person can mark one box without writing. A key under the heading says what the lowest and highest box mean, such as 1 = Poor, 5 = Excellent. Below the questions are lines for comments, and an optional row for a name, a phone number or email and the date.
The example has four questions with five boxes each, which is 20 boxes on the page. With the 10 box option the boxes are smaller so the row still fits. If you choose two forms per page, the page is split with a dashed cut line, and the page tells you when the settings are too long for two to fit.
Writing questions that work
Ask about one thing at a time. A question that asks about both speed and friendliness cannot be answered with one box. Keep the wording the same for every question so the scale means the same thing throughout, and say which end is better. The example questions are only examples to change.
Leave room for comments because the boxes tell you how people felt but not why. If you want to follow up with someone, add the contact line and say what you will use it for. Many people leave it blank, and that is fine. For a number you can track over time, see the Net Promoter Score Calculator, and for scoring candidates rather than customers, the Interview Scorecard Template.
Printing and privacy
The PDF is made in your browser and nothing you type is uploaded. If you tick the box to remember your entries, they are saved in this browser only, and unticking it deletes the saved copy. Print at 100 percent on US Letter paper. A two-per-page layout makes half-sheet forms you can cut along the dashed line. A full set of checklists, forms and handouts is in the Checklist Maker and the Permission Slip Generator.
Assumptions and limits
- The form is printed for people to fill in by hand. It collects nothing online and stores nothing on a server.
- Up to eight questions, up to eight comment lines and up to ten pages are supported.
- Long questions wrap onto a second line. Two forms per page need shorter settings, and the page says so when they do not fit.
- Questions and wording are yours. The example text is only an example.
Frequently asked questions
How do I make a customer feedback form?
Write a few clear questions, choose a rating scale with numbered boxes, add comment lines, and print. The generator above lays out the boxes and lines for you and gives you a PDF or a print from the browser.
How many questions should a feedback form have?
Fewer is usually better because people finish short forms. This tool allows up to eight. Four or five questions plus a comment area is enough for many small businesses, and you can change them to fit your own.
What rating scale should I use?
Choose 3 boxes for a quick good, fair, poor check, 5 for a familiar scale, or 10 for more detail. Whichever you use, say what the lowest and highest boxes mean, so every answer is read the same way.
Can I print two forms on one page?
Yes. Pick two forms per page and the page is split with a dashed cut line. The settings must be short enough for two forms to fit, and the page tells you if they are not.
