How to use the job application form template

      1. Enter your organization, a title and a short welcome note.
      2. Tick the sections you want: contact details, position, education, work history, skills, references and the confirmation.
      3. Set how many work history entries and references to print.
      4. Edit the statement above the signature, check the preview and print the form or save a PDF.

      What the form asks

The form asks for contact details, the position and when the person can start, the days and hours they can work, schooling and training, work history with duties and dates, skills and notes, and references. It ends with a confirmation line and a signature and date.

It does not ask about age, family status, religion, health, citizenship or other personal traits, and you should be careful before adding any questions. Keep every question tied to the job, and add role-specific ones, such as whether a candidate holds a license that the job requires.

      Keep it fair and consistent

Ask every applicant the same questions and review applications against the same criteria. A consistent form makes comparison easier, and an interview scorecard keeps the next step consistent too. What an employer may ask differs by place, so check local rules before you add questions.

After applications come in, use the Interview Scorecard Template to compare candidates, and when someone is hired use the New Hire Checklist Template. For a vendor rather than an employee, see the Vendor Information Form.

A blank example, not legal advice

This is an example template. Hiring law, what you may ask, how long you must keep applications and what wording you need for consent or background checks differ by place. Ask a qualified adviser to review the form before you use it. Everything is made in your browser, and nothing you type is uploaded.

      Assumptions and limits

      • The form is printed on US Letter paper and filled in by hand, and long forms continue on more pages.
      • The sections and the wording are examples you can change or remove.
      • No question about personal characteristics is included.
      • The company name in the sample is made up and is there to be replaced.

      Frequently asked questions

What should a job application ask?

The basics: contact details, the position, availability, education, work history and references. Keep questions tied to the job. Rules on what you may ask differ by place, so check yours.

Can I add my own questions?

Not on this page, which prints a fixed set of labeled lines. You can print the form and add a short extra sheet with questions that suit the role, after checking that they are allowed where you hire.

Why is there no question about age or health?

Many places limit what employers may ask about personal characteristics. The template stays with job-related items. Ask an adviser before adding any question beyond them.

How many work history entries can I print?

From 1 to 5, with room for the employer, title, dates, duties and the reason for leaving. The form continues over a second page if it needs it.
