      2. Choose how many log rows you want, up to 80.

      Formula

Log: Date, Who I spoke with, What was said or done, Follow-up due

Rows are ruled lines, and long logs continue on extra pages with the same columns

Checklist: one box and one ruled line for each document you list

The sheet is for your own records. It is not a claim form.

A roof claim with a 15 row log

With the claim number, insurer, policy number and date of loss filled in, the first page carries them under the title and then a ruled log of 15 rows. The default checklist lists photos or video, the incident report number, receipts, repair estimates, a list of lost items with values and copies of letters and emails.

If you ask for 40 rows the log runs onto a second page with its heading repeated, and the checklist always goes on its own last page. Leave the checklist box empty and the page is left out.

      Why keep a log

Claims can take weeks, with many calls, emails and letters. A dated log lets you say who told you what and when, which helps if you are given conflicting answers. Write the name of the person, a short note of what they said and any follow-up date. Keep copies of every document and photograph in one folder.

Note down each deadline the insurer gives you, such as the date by which an estimate must be sent. A follow-up column keeps those dates in front of you.

      Using the sheet

Print more than one copy if the claim is long, and keep the log with your documents. The sheet gives no advice about your policy or your rights. Read your policy and ask your insurer or an adviser if you are unsure. The Insurance Cancellation Refund Calculator and the Coinsurance Clause Penalty Calculator cover two related calculations. The Emergency Contact Sheet Printable holds important numbers, and the Bill Payment Tracker Printable tracks bills while the claim is open.

      Frequently asked questions

      Is this a claim form?

No. It is a log for your own notes. Use the forms your insurer gives you to make the claim itself.

How many rows can I print?

Up to 80 rows. Long logs continue on extra pages with the heading repeated.

Can I leave the claim details blank?

Yes. Blank lines are printed so that you can write the details by hand.

Does the date of loss need to be filled in?

No, it is optional. If you enter it, it is shown in a long format such as October 1, 2026.
